PROGRAM DESCRIPTION
S.T.A.R. HOUSE goal is to provide a comprehensive drug and alcohol rehabilitation services to recovering alcoholics/addicts and dual diagnosed individuals whose lives have become unmanageable as a result of drug addiction and alcoholism and introduce them to a new lifestyle free of chemicals. Treatment is provided through a social-therapeutic model and includes education, counseling, new lifestyle training, and introduction to the recovery community.
S.T.A.R. HOUSE is a long-term residential program that provides an opportunity for adult Men (18-65 years of age) to recover in a supportive peer environment for a period of twelve (12) months to two (2) years. This type of structured environment has proven to enhance efforts by individuals suffering from the disease of alcoholism and/or other drug addictions to recover and become productive members of society.
S.T.A.R. HOUSE extended stay feature of the program is designed to allow participants to go well beyond abstinence to learning how to make healthy, appropriate choices so that they may develop positive interpersonal relationships and can deal with issues that initially caused them to turn to illicit chemicals or alcohol for relief. Residents participate in a structure that teaches effective new living skills integrated with every aspect of family living. The therapeutic design includes such details as meal planning, deciding on appropriate support meetings to attend, planning weekend leisure activities, dealing with life issues of living in a shared house environment, and problem solving/conflict resolution.
S.T.A.R. HOUSE is a “working program” that teaches residents to be high functioning members of society. They are expected to secure full time employment thus enabling them to be responsible for ongoing treatment cost of $250.00 per week. The weekly fees include living quarters and utilities. Each resident is to provide his own food. In addition to the weekly treatment cost there is a entrance registration fee of $400.00 that is due upon admission into the program. This registration fee is non refundable. The initial fee to enter the program is $900.00. This is the NON-REFUNDABLE admission fee and the first two (2) weeks of treatment cost up front. This gives a new resident a two week window they are paid up and do not owe any money, while they are looking for employment.

Our drug and alcohol rehabilitation facilities, will offer a complete continuum of treatment to recovering alcoholic/addicts, and dual diagnosed individuals through outpatient treatment, sober living and aftercare.
The main objective of our program is to free individuals from the bondage of drug abuse and alcoholism. This program will help individuals to discover newer ways to live without the use of drugs and/or alcohol.
Every individual has a unique history to the disease. Hence, the drug rehab program must be tailor-made to suit each individual case.
Depending upon an applicant’s case, we will recommend the services that need to be included in the rehabilitation programs specifically designed for each individual.
Outpatient addiction recovery programs are a suitable option for the treatment of the disease at its early stage or if you have experienced a long period of drug abuse or alcoholism. This program will recommend a treatment plan for those individuals, whose occupational and family environments are intact as well as those who are presently without occupation or family environments who demonstrate a high degree of commitment to quit using drugs and alcohol. Our program provides adequate support service for day-to-day life.

Program Phases:
The substance abuse program designed by S.T.A.R. HOUSE consists of four (4) phases. The first month is considered the transition phase followed by three (3) additional phases over an eleven (11) month period. Residents are required to earn their way through the phases by the continued display of productive behaviors. There is also a phase four (4) category for residents to continue past the twelve (12) month program. Completion certificates are awarded only to those individuals who are able to complete the program requirements and display the appropriate ability for independent living. The program phases are described as follows:
Transition Phase
All residents must comply with rules and policies of the S.T.A.R. HOUSE. Curfew, bedtime, employment, visitation, sponsorship, involvement with a home group, and complying with the buddy system are areas of policy with rules.
Curfew is 7:00 p.m. seven (7) days per week. The only exception allowed is that residents may attend local 12-Step Meetings at 8:00 p.m. Residents are expected back no later than 9:30 p.m. Bedtime and lights out are at 11 p.m. Sunday through Thursday; 12 Midnight on Friday and Saturday nights.
Residents are to have a full-time job within two (2) weeks of their entry date. Residents are allowed to work 1st, 2nd, and 3rd shifts as approved by staff. All employment has to be approved by the S.T.A.R. HOUSE staff. Self-employed residents will have to show a record of the business history to be allowed to continue working in their business. If within three weeks of entry to the program, a resident has not secured a full time job, the resident will participate in a meeting with staff to determine if discharge is the appropriate consequence.
Job searches will take place between the hours of 9:00 a.m. and 5:00 p.m. six (6) days per week until job has been obtained. A “Job Search” form will be completed daily. House Managers will sign this form to verify that a job search was completed that day.
No Resident in the Transition Phase is allowed to leave the facility alone with the only exception being related to a job search or work.
No visitation is allowed during Transition Phase.
A Sponsor is to be acquired by resident within two (2) weeks after entry to program. Residents are required to contact selected Sponsor a minimum of five (5) days per week. At least one of the weekly contacts must be a face-to-face working meeting.
Residents must obtain a “home group” in the 12-Step Community to gain Phase One status.
Phase One
Residents are eligible for Phase One after completing thirty (30) days in the program. Entry into Phase One requires full time employment (minimum of 32 hours per week). Residents must comply with Phase One guidelines, specifically the regulations and policies concerning curfew, bedtime, employment, visitation, and complying with the buddy system. Phase One is designed for a more intense level of program as they become more accustom to their surroundings and their peers.
Curfew is 7:00 p.m. seven (7) days per week. The only exception allowed is that residents may attend local 12-Step Meetings at 8:00 p.m. Residents are expected back no later than 9:30 p.m. Bedtime and lights out are at 11 p.m. Sunday through Thursday; 12 Midnight on Friday and Saturday nights.
When leaving the housing facility, the Resident must be accompanied by another Resident who currently has the privileges of Phase Two status. The exception to this rule is going to and from a place of employment.
Phase One Residents may be signed out by a Recovering Resident who has Phase Three status and has maintained six months of continuous sobriety.
Phase One Residents are eligible for visitations from immediate family only. Family visitations are Saturday and/or Sunday from 1:00 p.m. to 5:00 p.m. and must be requested 7 days in advance.
If a Phase One Resident loses a job, he will be given three weeks to follow the job search policy to obtain another job. If a replacement job has not been located, he will participate in a meeting with staff to determine if discharge is the appropriate consequence.
Cell phones are allowed in Transition and Phase One only if Resident is in total compliance to that phase’s requirements.
Family member are not allowed to transport Residents to employment or meetings.
Phase One Residents must write a full descriptive paper about step 1 of the 12 Step model or have a full descriptive discussion with Phase Three Resident and Staff. A peer review will take place prior to advancement to Phase Two.
Phase Two
At the completion of 60 days of residency and subsequent to the 30 day transition up to Phase One, Residents are eligible for Phase Two status.
Curfew for Phase Two Residents is 10:00 p.m. Sunday through Thursday, and 11:00 p.m. Friday and Saturday.
Also a 24-hour weekend pass is available for visits with family at pre-approved addresses (if the family environment is constructive to our plan of abstinence). Family visitation privileges also continue for the weekends (Saturday and/or Sunday) between the hours of 1:00 p.m. and 5:00 p.m. All Residents must be back from pass prior to the Community Peer Meeting each Sunday. Visitations and Passes must be requested at least seven (7) days in advance.
Phase Two Residents have the added freedom to move within the Gainesville Community without being accompanied by other Residents. They do have the responsibility to accompany Phase One Residents in their travels outside of the transitional living facility.
Driving privileges will be granted with presentation of proper documentation.
Phase Two Resident will lose their status if program payments are not current and they are not in total compliance with program expectations (employment, attitude, absences or tardiness to meetings, curfews, house chores, etc.) Re-entering Phase Two will be decided during the Community Peer and Staff Meeting.
Phase Three
Residents are eligible for Phase Three status after the successful completion of six (6) months in the program. Staff and fellow Phase Three residents review the individual’s history in the program to decide whether the Resident has transitioned into recovery. Considerations are progress level at the time of review, along with full compliance with program expectations, rules, and regulations. These are vital to this decision.
Phase Three Residents assist the Clinical Director in decision-making events that affect the community as a whole. The Phase Three Residents will model their recovery and present positive examples to the New Admissions. The Staff and Phase Three Residents become more interactive as they advance to this phase of their recovery.
Curfew for Phase Three Residents is 11:00 p.m. Sunday through Thursday, and 12:00 a.m. Friday and Saturday.
Also a 24-hour weekend pass is available for visits with family at pre-approved addressed (if the family environment is constructive to our plan of abstinence). Once every 30 days Phase Three Residents are allowed a 48-hour weekend pass. All Residents must be back from pass prior to the Community Peer Meeting each Sunday. Visitations and Passes must be requested at least seven (7) days in advance
Phase Four
Once a Resident has completed twelve (12) successful months in the program, they will be eligible for Phase Four. Individuals who have shown remarkable progress may wish to continue the program for a longer period. These Residents must be in full compliance with current 12-Step responsibilities.
Phase Four Residents must have completed work on the 7th step with their Sponsor and be current with program payments. Benefits for these individuals include no sign-out or sign-in requirements.
Also a midnight curfew seven days a week, computer privileges in their room, and 48 hour weekend passes are available upon request as well as a seven (7) day vacation with immediate family.
Admission Criteria:
The main criteria for entry into the S.T.A.R. HOUSE program is the applicant must be able to pass a drug screen. The S.T.A.R. HOUSE will accept individuals who have been diagnosed with a mental health disorder without psychotic features. Participants in this program must be able to function successfully in a structured living environment without a threat of being a danger to self or others.
If a Resident develops behaviors during periods of abstinence that suggest a mental health disorder exists, they will be referred to Avita Partners Health Services located in Hall County to determine if medication is appropriate.
Staff Training/Qualifications:
The Director and Manager of the S.T.A.R. HOUSE has the training and experience to identify and treat self-destructive behaviors that can contribute to an actual relapse. Staff examine the behavior of each resident to assist them in recognizing certain changes to the desire to re-enter the maladaptive behaviors associated with alcoholism and/or drug addiction. This method of relapse prevention is extremely effective. Self identification of stressors, self-destructive behaviors and mental health disorders play an important role to the recovery of each addict. The Director has completed 180 Hour Basic Substance Abuse Counselor Certification Course which includes specific training in Client Systems, Assessment Interviewing & Treatment Planning, HIV & AIDS, Ethics, and Cultural Diversity. Director also completed twenty (20) hours of training as Relapse Prevention Specialists.
